Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility.

The PACE LPN provides general nursing care to PACE participants, executes nursing interventions under the supervision of the PACE primary care providers and RN nursing leadership staff, and collects data on participant medical conditions in the PACE clinic and by telephone. The PACE LPN provides nursing education to participants, caregivers, and PACE staff. The PACE LPN is responsible for scheduling participant appointments for PACE Clinic referrals made to medical specialists and ancillary care providers in the community.

Work Hours
- Full-time position: 80 hours bi-weekly
- Day shift
Job Requirements
Graduate of an approved technical, professional, or vocational program in Nursing. (Required)
1 - 2 years of experience working with a frail or elderly population or, in the absence of such experience, receive appropriate training from PACE on working with a frail or elderly population before providing direct patient care to PACE participants independently. Relevant experience must be within the last 3 years. (Required)
3 - 4 Years Of LPN Clinical Experience. (Preferred)
Other Current LPN license in good standing to practice in Ohio. Upon Hire Required
Basic Life Support (BLS) Required
Job Responsibilities
Under the supervision of the primary care provider and PACE nursing leadership staff, provides nursing interventions and completes PACE participant nursing interventions in the PACE Clinic or community setting, and escalates any nursing concerns to the primary care provider and PACE nursing leadership. Serves as a nursing subject matter expert and communicates best practices for a frail elderly population to the IDT, participants, and caregivers.
Responsible for scheduling participant appointments for referrals to medical specialists and ancillary care providers in the community. Responsible for entering instructions in the Electronic Health Record (EHR) for medical specialists and ancillary care provider appointments in the EHR. Documents nursing interventions in participant care plans and the PACE EHR.
Administers participant medications in the PACE Clinic as directed by the primary care provider and PACE nursing leadership staff.
Reviews, signs off, and disseminates nursing orders and instructions to appropriate PACE staff and PACE contracted personnel.
Provides care coordination during care transitions and proactively communicates with hospital and long-term care facility case managers regarding participant discharge care plans.
Monitors PACE participants for changes in their health or psychosocial status. Promptly communicates pertinent changes in participant status to the IDT.
Attends IDT and morning meetings and reports on participants’ clinical status as directed by PACE nursing leadership staff. Participates in the nursing on-call rotation to provide after-hours and weekend call coverage to PACE participants. Supports the PACE QI program and participates in PACE QI activities as assigned.
