Job summary

Renew Health + Recovery provides in-home and virtual substance use disorder treatment, bringing personalized care directly to clients where they live. You will deliver IHAT services, collaborate with a multidisciplinary team, and manage your own client caseload while maintaining timely documentation.

The role requires personal recovery experience, a valid license, and willingness to work nights or weekends.

Qualifications

• Must actively reside within 30 miles of Kokomo, IN area.
• Valid driver's license, auto insurance and reliable transportation.
• Minimum 2 years lived recovery from SUD or related personal experience.
• High School Diploma or GED required.
• Peer recovery licenses (CAC, DARC, RCA, CPRS, CRSW) preferred but not required.

Responsibilities

• Deliver IHAT in the client’s home and community environment.
• Collaborate with addiction treatment team to coordinate client care.
• Independently manage client caseload and appointments.
• Submit clinical documentation in an EMR system.
• Participate in case reviews, supervision, team meetings and continuing education.
• Be open to nights and weekends when required.

Job description

• Deliver Renew Health + Recovery's in-home addiction treatment (IHAT) program within the client’s home and community environment.
• Collaborate with the addiction treatment team to coordinate individualized, holistic client care
• Independently manages client caseload and appointments
• Submit timely clinical documentation in an Electronic Medical Record (EMR) system
• Demonstrate understanding of stages of change and ability to incorporate them into client care
• Provide personal experience and guidance to clients through early and sustained recovery
• Develop wellness recovery action plan (WRAP) with client; coordinate with team regarding treatment planning
• Participate in case review, supervision, team meetings, SWAT, continuing education, and any other meetings identified by the team
• Must be open and flexible to working nights and weekends, when required
• Perform miscellaneous job-related duties as assigned

Qualifications:

• Must actively reside within a 30-mile radius of the Kokomo, Indiana area.
• Reliable transportation along with a valid driver's license and valid auto insurance
• Minimum of 2 years of lived experience in continuous recovery from any substance use disorder (SUD) and/or personal experience with addiction in family members or loved ones
• High School Diploma/GED required
• CAC , DARC , RCA , CPRS, CRSW, or other state Peer Recovery Specialist license is preferred but not required
