Pay Range

This range is provided by Homes for the Homeless. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more.

Base pay range: $70,000.00/yr - $70,000.00/yr

Role Overview

We are seeking a passionate and experienced Client Care Coordinator to join our team at Homes for the Homeless (HFH). The role is based at our Families w/ Children facility in Williamsbridge, Bronx. You will provide mental health and related services to homeless families, conduct thorough assessments, collaborate with case management staff, and establish community linkages to support family success. About Homes for the Homeless

HFH operates shelters for families experiencing homelessness in NYC. Our residences offer onsite programs to help families thrive long after they leave our shelters.

Responsibilities
  • Conduct comprehensive bio-psychosocial assessments with families.
  • Lead and coordinate social services and counseling support.
  • Ensure effective delivery of social services, incorporating trauma-informed and family-centered care.
  • Monitor and document client progress toward goals.
  • Support families in shelter and refer for additional services as needed.
  • Collaborate with the Family Services team and support staff.
  • Enhance linkages to mental health, education, and community services.
  • Improve permanency outcomes for families with children.
  • Manage record keeping, data collection, and service evaluation.
  • Assist with unit inspections and staff trainings.
Qualifications
  • Master’s degree in Social Work or Mental Health Counseling.
  • Licensed LMSW or LMHC in New York.
  • Knowledge of substance use disorders, trauma, domestic violence, child abuse, and mental health conditions.
  • Sensitivity to working with homeless and vulnerable populations.
  • Bilingual skills are a plus.
Compensation & Benefits
