Comfort Keepers is seeking an External Client Care Coordinator to connect with individuals and families, guide them through home care options, and ensure a smooth onboarding process.

Responsibilities

• Connect with individuals and families to discuss their home care needs and guide them through available services
• Establish and maintain relationships with healthcare providers, community organizations, and referral partners
• Visit clients to ensure they are receiving outstanding care and address any questions or concerns
• Partner with caregivers and office staff to coordinate services and provide a positive client experience
• Support new client onboarding and help ensure a smooth transition into care
• Participate in community outreach activities and represent Comfort Keepers at local networking events
• Keep client information organized and maintain accurate records
• Identify ways to improve client satisfaction and strengthen long-term relationships
• Communicate regularly with families and the care team to ensure quality service
• Represent Comfort Keepers with professionalism, compassion, and integrity

Service Areas: Cleona, Fredericksburg, Lebanon, Myerstown, Annville, Palmyra, Hershey, Manheim, Elizabethtown, Mount Gretna, and surrounding communities.

What You'll Do
