Job summary

South West Georgia Homecare seeks a scheduling and care coordination professional to own the day-to-day client experience. You will run intakes, match caregivers to families, and build the weekly schedule, serving as the families' direct point of contact.

Responsibilities include completing intakes and in-home assessments, assigning caregivers by skills and proximity, and coordinating hospital discharge and referrals while documenting changes in real time.

Qualifications

• Two or more years of scheduling, care coordination, or healthcare operations experience.
• Excellent phone and written communication with families.
• Comfort working in scheduling and electronic record systems.
• Strong organization under shifting daily priorities.
• Knowledge of Dougherty, Lee, and surrounding counties preferred.
• Ability to pass background screening.
