Wheeler Clinic is seeking a Client Care Coordinator to liaison with DOC/CSSD referrals and coordinate communication between the agency and referral sources. The role includes assisting uninsured clients with Medicaid applications and managing data entry for program reporting.

The position requires attention to detail, strong communication, and ability to maintain accurate records while supporting client engagement and service access.

Qualifications

• High School diploma with 2 years of progressively responsible work experience.
• Strong written and verbal communication skills.
• Detail-oriented with thorough follow-through and professional demeanor.

Responsibilities

• Serves as liaison between Wheeler Clinic and the DOC/CSSD referral source.
• Ensures timely completion of all DOC/CSSD-related correspondence, reporting and data entry.
• Assists uninsured clients with Medicaid applications and eligibility processes.
• Coordinates with clinical staff, referral sources and clients to support engagement and retention.
• Maintains data in CDCS and tracks program requirements as needed.
