Job summary

Wheeler Clinic seeks a Senior Counselor to provide direct services, case management, and care coordination for an assigned caseload. You will liaise with families, DCF, schools, probation, and other supports to ensure effective services and advocacy.

Qualifications include a BA/BS in Psychology, Social Work, Counseling or related field, plus experience with mental health/substance use populations. Valid driver’s license required and a commitment to comprehensive, patient-centered care.

Qualifications

• BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling or related field is required.
• Education may be substituted with related experience on a year for year basis.
• Previous experience working with client population with mental health and/or co‑occurring substance use disorders.
• Effective time management, strong verbal and written communication skills are required.

Responsibilities

• Develops case management plans and coordinates care for assigned clients and families.
• Provides front line crisis support by phone, in home or in the community.
• Conducts counseling with individuals, families, and groups as appropriate.
• Maintains documentation for client records in compliance with Medicaid and licensing.
