Sevita’s ResCare Community Living in the United States seeks a dedicated Case Manager to support individuals with intellectual and developmental disabilities at home and in the community. You will conduct intake and assessments, coordinate services, and advocate for person-centered plans that promote independence.
The role emphasizes crisis intervention, collaboration with health and social care professionals, and developing comprehensive care plans to maximize living outside institutions.
