Job summary

NeuroRestorative, part of Sevita, seeks a Case Coordinator to organize admissions, direct participant care, and coordinate internal/external services in a medical/rehab setting. You’ll develop individual programs, liaise with families, chair conferences, and ensure proper documentation and discharge planning.

Responsibilities

  • Develop and coordinate individual participant programs to meet needs.
  • Assist participants in understanding goals and processes.
  • Act as liaison for families with information and resources.

  • Review reports for consistency and proper documentation.
  • Discharge planning coordination and follow-up.
  • Coordinate internal communications on case-specific issues.
  • Attend clinical team and supervision meetings.
  • Review monthly bills for discrepancies with accounting.
  • Coordinate incentive accounts with Case Manager.
  • Arrange internal medical appointments and ensure timely completion.
  • Document summaries and tracking information for appointments.
  • Oversee healthcare planning with Program Director/Manager.
  • Coordinate with community resources to ensure quality care.
  • Assist in training new staff involved in medical care.

Job description

NeuroRestorative, a part of the Sevita family, provides rehabilitation services for people of all ages with brain, spinal cord and medically complex injuries, illnesses and other challenges. In a variety of locations and community-based settings, we offer a range of programs, including vocational and therapy programs, day treatments, and specialized services for infants, children, adolescents, Military Service Members and Veterans.

Case Coordinator

Pay is 17.25-18.25 an hour

As Case Coordinator, you will be responsible for providing organizational assistance and program support, including admissions procedures, direct participant care, and coordination of internal and external services.

  • Develop individual programs and assure that the program meets the participant’s needs, optimizing the participant’s potential to be integrated into the community; ensure the provision of appropriate services to designated program participants
  • Assist designated program participants, as needed, to understand the overall goals and processes being implemented in the programs
  • Act as a liaison for families and provide them with information and resources as requested
  • Organize and chair program participant conferences
  • Review the designated program participant's reports (evaluation, progress, and discharge) for consistency and coordination of goals; ensure the appropriate documentation/recordkeeping of designated program participants
  • Responsible for development, coordination, implementation, and follow-up of discharge planning as directed
  • Assist in internal communication to team members regarding case-specific issues
  • Attend required meetings, including clinical team meetings and regularly scheduled supervision meetings
  • Review monthly bills for designated program participants after receipt from accounting staff; identify any discrepancies to accounting staff
  • Coordinate utilization with the Case Manager relative to program participant’s incentive account
  • Organize and facilitate internal medical appointments as requested by management, overseeing timely completion of annual medical appointments
  • Ensure the appropriate documentation (including summaries and tracking information) of designated program participants for internal medical appointments and distribute as required
  • Direct, develop, coordinate, implement, and follow up on healthcare planning as directed by the Program Director/Manager; ensure appropriate medical follow-up for designated program participants
  • Communicate with community resources and persons outside of the program to assure quality program participant healthcare for all assigned program participants
  • Assist in training new staff involved with overseeing medical care as requested
Qualifications
  • High School diploma or equivalent; Associate or Bachelor's Degree in Social Service-related field preferred
  • Two years of direct or indirect support services to persons with disabilities in a medical and/or rehabilitation setting
  • Strong attention to detail, organizational skills, and the ability to multi-task to meet deadlines
  • Effective communication skills to manage relationships
  • Self‑motivated and collaborative; a team player
  • A reliable, responsible attitude and a compassionate approach
