Responsibilities

• Provide dementia care, including ADLs, meaningful engagement, and communication, following person-centered plans.
• Assist residents with daily activities, encouraging independence and respecting individual needs.
• Participate in team meetings and know residents' life stories, preferences, and routines.
• Serve meals, promote proper nutrition, and record changes in eating habits.
• Lead engagement programs and promote socialization among residents.
• Maintain accurate records of care, medication, and observations.
