Description

Provides dementia care including adapted ADLs, meaningful engagement, and communication to residents following a person-centered individual service plan. Treats residents with respect and dignity, recognizes individual needs, and encourages independence. Fosters a purposeful and engaging culture throughout the community.

  1. Assists residents and provides input regarding the care plan and daily living activities, including bathing, dressing, grooming, toileting, transferring, and attending programs and meals according to the individual service plan and interventions. Partners and encourages residents to be as independent as possible, promoting self-care.
  2. Participates in team behavioral expression meetings or discussions with community leadership.
  3. Knows residents' Life Story, preferences, routines, and interests.
  4. Serves meals in the dining room and may assist in meal preparation following dementia-friendly menus. Encourages residents to eat properly and stay hydrated, respecting their right to free choice. Reports changes in eating habits to the supervisor.
  5. Initiates or leads at least one Daily Path engagement program, encouraging socialization and participation in person-centered activities, fostering friendships among residents.
  6. Maintains residents' records daily, including documentation related to medication, engagement interests, incidents, observations, and changes in physical or emotional condition, in accordance with company policies.
  7. Observes and reports residents' health and emotional status, following medication policies and state regulations when assisting with medication, if permitted.
  8. Maintains a clean, safe environment, performing housekeeping tasks and following cleaning schedules.
  9. Responds promptly and appropriately to emergency situations and resident requests, including monitoring resident location and notifying supervisors if residents are unaccounted for.
  10. Encourages teamwork and cooperative interactions with co-workers.
