Description

Provides dementia care including adapted ADLs, meaningful engagement, and communication to residents following a person-centered individual service plan. Treats residents with respect and dignity, recognizes individual needs, and encourages independence. Fosters a purposeful and engaging culture throughout the community.

1. Assists residents and provides input regarding the care plan and daily living activities, including bathing, dressing, grooming, toileting, transferring, and attending programs and meals according to individual plans and interventions.
2. Partners with and encourages residents to be as independent as possible, supporting self-care efforts.
3. Participates in team behavioral expression meetings or discussions with community leadership.
4. Knows residents' life stories, preferences, routines, and interests.
5. Serves meals in the dining room and may assist in preparing dementia-friendly menus, encouraging proper diet and fluid intake while respecting residents' rights to free choice.
6. Records and reports changes in eating habits to supervisors.
7. Initiates and/or leads at least one daily engagement program based on residents' interests and routines, promoting socialization and participation.
8. Maintains residents' records daily, including documentation related to medication, engagement, incidents, observations, and changes in physical or emotional condition.
9. Assists with medication administration if permitted by state regulations, following physician orders and policies.
10. Maintains a clean, safe, and orderly environment, performing housekeeping tasks as scheduled.
11. Responds promptly to emergency situations and resident requests, following proper procedures.
12. Monitors residents' locations and reports any inability to locate a resident immediately.
13. Encourages teamwork and cooperative interactions with co-workers.
