A leading senior living community seeks a Clare Bridge Caregiver dedicated to providing compassionate dementia care and supporting resident independence. This role involves assisting residents with daily activities, promoting social engagement, and maintaining documentation. Clare Bridge Caregiver (Finance)
Provides dementia care including adapted ADLs, meaningful engagement, and communication to residents following a person-centered individual service plan. Treats residents with respect and dignity, recognizes individual needs, and encourages independence. Fosters a purposeful and engaging culture throughout the community.
Assists residents and provides input regarding the care plan and daily living activities, including bathing, dressing, grooming, toileting, transferring, and attending programs and meals according to the individual service plan and interventions. Partners with residents to promote independence, encouraging them to perform as much of their own care as possible.
Participates in team behavioral expression meetings or discussions with community leadership. Knows residents' Life Stories, preferences, routines, and interests.
Serves meals in the dining room and may assist in meal preparation following dementia-friendly menus. Encourages residents to eat properly and stay hydrated, respecting their right to free choice. Reports any changes in eating habits to the supervisor.
Initiates or leads at least one Daily Path engagement program based on residents' Life Stories or programs like Solace. Encourages socialization and participation in person-centered activities and supports friendships among residents.
Maintains residents' records daily, including documentation related to medication, interests, incidents, observations, and health changes, in accordance with company policies. Reports any physical or behavioral changes promptly.
Assists with medication administration if permitted by state regulations, following physician orders and laws. Exhibits understanding of medication policies and procedures.
Maintains a clean, safe, and orderly environment, including housekeeping, laundry, and sanitation schedules.
Responds promptly to resident requests and emergencies, following proper procedures. Monitors residents' locations and reports if a resident cannot be located.
Encourages teamwork and cooperative interactions with co-workers.

Salary range: $12.84 - $16.04. Role may include bonus opportunities. Benefits eligibility varies by employment status.
