Qualifications

Responsibilities

• Utilize evidenced-based health coaching interventions to improve patient engagement and outcomes.
• Guide patient through the healthcare provider created treatment plan.
• Collaborate with the Care Team to develop integrated treatment plans.
• Provide behavior change coaching and care to a full patient panel.
• Conduct virtual, synchronous treatment sessions with patients.
