Job summary

Arcare in Little Rock, AR is seeking a Chronic Disease Manager RN/LPN to provide education and case management for primary care patients with chronic diseases, including HIV. You will work with care teams and community resources to ensure personalized, comprehensive care that improves health outcomes.

The ideal candidate will demonstrate strong communication and organizational skills, navigate complex healthcare needs, and contribute to a collaborative team dedicated to Health for All.

Qualifications

• Associate degree required; Bachelor’s degree preferred.
• Current RN or LPN license in the appropriate state required.
• Minimum of 1 year of experience in a medical facility; case management experience preferred.
• Proficiency in Word, Excel, and electronic record-keeping systems.
• CPR and TB certification renewed annually.

Responsibilities

• Develop individualized Medical Services Plans for patients within 30 days of enrollment.
• Provide ongoing case management and education on chronic disease management.
• Coordinate with healthcare providers and community resources to support patient care.
• Conduct patient monitoring via phone, telehealth, secure messaging, or in-person interactions.
• Document patient interactions and care plans in CAREWare and other systems.
• Participate in case conferences, outreach events, and training sessions.
• Educate patients and families about self‑management goals and available support services.
• Collaborate with federal, state, and local agencies to maximize resources and achieve program goals.
• Support team members with patient care tasks, quality improvement initiatives, and special projects.
