Job summary

Teche Health in Franklin, Louisiana, seeks a Chronic Care Management Coordinator to oversee care for adults and children with complex illnesses in a primary care setting. The role reports to the Chronic Disease Project Manager and focuses on optimizing patient health through evidence-based plans.

You will lead care management initiatives, train staff, and coordinate with specialists to ensure high-quality, cost-effective care delivery while improving patient outcomes.

Qualifications

• LPN license required with current active license.
• At least two years’ experience with adult and pediatric patients in primary care or ambulatory settings.
• Knowledge of chronic conditions and care management principles.
• Strong communication, triage, and patient education skills.
• Ability to use data and evidence-based guidelines to develop care plans.

Responsibilities

• Identify CCM population and assess needs over time.
• Collaborate with PCP and care team to develop individualized care plans.
• Provide self-management support to empower patients and families.
• Coordinate care across settings including hospitals and post-acute services.
• Maintain documentation and participate in QAPI/quality improvement.
• Assist with training and process improvements within the practice.
