Job summary

Vitability Health is seeking an experienced Case Manager to oversee a panel of chronic care patients, including those with mental health needs. You will collaborate with physicians, providers, and care staff to identify patients for integrated care plans and ensure timely follow-up.

The role focuses on promoting self-management, coordinating referrals, documenting progress, and participating in team meetings to improve outcomes.

Qualifications

• Knowledge of the Patient-Centered Medical Home model/mission.
• Understanding of insurance industry practices and requirements.
• Understanding of chronic disease and preventive care measures.
• Bachelor’s degree in health care administration, health informatics, or related field and licensure as LPN; RN preferred.

Responsibilities

• Manages a caseload of chronic care patients.
• Collaborates with physicians, providers, and staff to identify appropriate patients for care management.
• Develops relationships with patients as an integral team member.
• Provides follow-up management to ensure adherence to care plans.
• Maintains telephone availability and handles urgent calls.
• Coordinates referrals and assists with transitions of care.

Education

Bachelor's degree in health care administration, health informatics, or a related field

LPN or RN licensure preferred
