Job summary

Catholic Charities Family and Community Services (CCFCS) is hiring a full-time Intake and Care Management Specialist to serve Monroe, Finger Lakes, or Southern Tier regions. The role requires a Bachelor’s degree, NY driver’s license, and 2–4 years experience with disabilities or chronic illnesses.

Qualifications

• Education: Bachelor’s Degree
• Credentials: Valid NY State Driver’s License; driving record acceptable by agency/insurer
• Bilingual (English/Spanish) required for some positions; additional compensation offered for bilingual skills
• Experience: 2–4 years working with individuals with disabilities or chronic illnesses
• A combination of education and experience may be substituted at the Director’s discretion

Responsibilities

• Serve as intake and care management contact and conduct necessary screening for intake in applicable programs
• Provide client-guided care management to assigned caseload with accurate documentation
• Complete comprehensive assessments, crisis plans, and care plans per DOH standards
• Demonstrate cultural competency and respect for clients
• Support and encourage diversity and strengths of clients
• Provide monthly core services to address care plan goals and client satisfaction
• Maintain team accessibility for clients during workday
• Assist clients with obtaining public benefits (Medicaid, SNAP, etc.)
• Coordinate resources for medical, mental health, social, housing needs
• Remain current in field and implement best practice service models
• Make independent risk assessments and provide crisis intervention as needed
• Be available to clients in homes/communities, including outside hours
• Provide on-call availability for Health Home program
• Advocate for clients and develop Care Team through regular meetings
• Participate in staff meetings and training
• Ensure compliance with laws, regulations, and agency policies
• Engage in ongoing professional development
• Represent CCFC S on boards/committees as assigned
• Participate in agency meetings/trainings
• Pursue development of professional competencies
• Align with Agency Mission, Vision, Values and Strategic Plan
