Job summary

SuperCare Health is hiring for a Telecare Care Coach in Arizona to support Chronic Care Management programs. You will handle patient demographics, coordinate home visits, and maintain accurate records while delivering exceptional customer service.

This hybrid role requires 2–3 days in the office and involves scheduling meetings, responding to inquiries, and ensuring compliance with policies. A High School diploma and 3+ years in customer service within managed care are expected.

Qualifications

• High School diploma and three years of customer service in a managed care setting.
• Experience with Medicare Advantage enrollment activities is preferred.
• Proficient in MS Office (Word, Excel, Access) and data entry.

Responsibilities

• Enter new patient demographics and prepare delivery kits.
• Schedule initial RT home visit, appointments and equipment delivery; maintain census.
• Review patient information for accuracy and completeness; coordinate imaging and related documents.
• Maintain program information in the electronic system and equipment logs.
• Schedule and support meetings; record meeting minutes and distribute agendas.
