Job summary

Optima Medical in Arizona is seeking a Care Coordinator for our in-office, phone and EMR-based coordination program. You will work directly with patients living with chronic conditions to help them stay on track with their care plans through regular outreach and follow-up.

This role is ideal for Medical Assistants seeking the next step in their career while applying clinical knowledge daily. EMR experience (eClinicalWorks preferred), strong communication, professionalism, and bilingual

Qualifications

• At least 1 year of clinical work experience as a Medical Assistant.
• At least 1 year of EMR experience; eClinicalWorks preferred.
• High level of professionalism.
• Excellent written and verbal communication skills.
• HS diploma or GED equivalent.

Responsibilities

• Develop personalized care plans with patients, families, and providers.
• Coordinate communication among physicians, nurses, and ancillary staff.
• Monitor adherence to treatment plans and track progress.
• Provide education on disease management and preventive care.
• Document encounters in EHR and ensure continuity of care.
• Connect patients with community resources to address social determinants.
• Conduct regular follow-up checks to optimize health outcomes.
