Job summary

VSHP Volunteer State Health Plan, Inc is hiring a CHOICES Care Coordinator to visit members at home or in community settings, coordinating services to improve health and independence. You will partner with families, assess needs across physical, behavioral health, and social domains, and help navigate resources to reach personal goals.

The role requires a TN RN license or other eligible licensure, 3+ years clinical experience, and Shelby County residency with a valid driver’s license.

Qualifications

• RN license active in Tennessee or license in the state of residence if participating in the Nurse Licensure Compact Act, or Masters in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW).
• 3 years of experience in a clinical setting.
• Excellent customer service, time management, and communication skills.
• Must live within Shelby County, TN and have a valid driver’s license.

Responsibilities

• Partner with members and families to identify needed supports and direct services to meet health goals.
• Collaborate with clinical and social support teams to meet physical, behavioral health, and long-term service needs.
• Conduct thorough face-to-face visits and assess the member's situation to determine status and needs.
