Job summary

NATURE OF WORK

The primary goal of Wraparound Facilitation is to support the child and their family strengths, promote community involvement, provide needed resources to families to improve the family well-being and reduce risk of child abuse/neglect. They work closely with service providers to help assure that services and clinical treatment modalities augment each other for optimal outcomes for the child and their family. The WF will lead the treatment team through engagement and team preparation, plan development, plan implementation, and transition, and provide intensive wraparound facilitation as per the National Wraparound Initiative standards.

ESSENTIAL DUTIES
  • Engages and communicates effectively and regularly with youth and their families/legal guardians, and all team members.
  • Collaborates and plans effectively to access the needed supports and services for each youth and family served by working with youth and family-serving agencies and organizations in a community.
  • Conducts home/site visits with the youth and their family in accordance with the need’s determination from team members.
  • Schedules appointments and assures all parties have relevant consents and information needed.
  • Completes the CANS and other screeners or assessments as indicated for each youth/family served at designated intervals and includes outcomes in the youth’s individualized care plan.
  • Documents all activities per policy and ensures documentation is completed in a timely manner as per company policies.
  • Assures an individualized care plan for each youth/family is developed by a team of people who are relevant to the youth and family to achieve their goal of having adequate community resources and supports.
  • Complete intake assessments for new CSED clients, as well as be trained and available to assist with walk in clinic intake assessments as needed.
  • Coordinates all wraparound plan meetings by scheduling the meeting and notifying all team members in a timely manner.
  • Ability to assess and respond effectively to crisis situations.
  • Participates in on call for youth and families in wraparound program.
  • Advocates for youth and their families by attending team meetings, court hearings, IEP/SAT meetings, etc.
  • Prepares effectively for youth and their families to transition from wraparound services to least restrictive care management.
  • Adheres to confidentiality, HIPAA, and risk management policies and procedures including but not limited to completing Incident Reports.
  • Attend trainings as needed to ensure quality service & fidelity to wraparound model.
  • Use electronic conferencing technology, such as Zoom, Microsoft Teams, and Conference Call features on a Telephone to facilitate consumer interaction when face-to-face is not possible.
  • Completes and submits Electronic Medical Record documentation as required by Valley, OHFLAC, BMS, CMS, and other licensing agencies (Notes Including Direct Support Progress Notes, Master Treatment Plans, etc.) completed accurately and timely and will accurately and appropriately reflect services the consumer(s) has received.
  • Meets direct consumer service time (DCST) standards set forth.
  • Adheres to Valley’s Values & Standards of Performance & Ethical Conduct.
  • Other work-related duties as assigned.
Qualifications
MINIMUM QUALIFICATIONS
  • Ability to perform Essential Duties as outlined below.
  • BA/BS in Human Service Field.
  • 1 to 2 years’ experience with Mental Illness & Behavioral Health Diagnoses.
  • Possess knowledge of mental illness diagnoses and behavioral disorders in children.
  • Flexible with schedule in order to meet with youth and family wraparound team members.
  • Ability to collaborate closely with the youth's team and family members to develop and implement the individualized plan of care.
  • Ability to comply with all of Valley’s Policies and procedures.
  • Ability to comply with Consumer’s Rights.
  • Ability to comply with Valley’s and Division departmental safety procedures.
  • Ability to read, write and speak the English language.
  • Valid Driver’s License.
  • Ability to obtain and maintain CPR/First Aid, and MANDT Certifications.
PREFERRED QUALIFICATIONS
  • Previous Experience in the MI & Behavioral Health Field for Children
  • Previous Experience working in CSED, ACT, or other Wraparound Programs
  • Previous Experience working in Service Coordination
