Job summary

Mountain Valley Developmental Services is seeking a Service Coordinator for the Children’s Program. The role involves coordinating services, facilitating IFSP meetings, and advocating for families to access resources.

This is a full-time position with a 40-hour work week and a focus on early intervention services. The coordinator will work closely with families, providers, and community partners to develop individualized programs and monitor progress in accordance with regulatory guidelines.

Qualifications

• BA/BS in education, psychology, or related field preferred.
• Experience coordinating services for families and children in developmental services.
• Familiarity with IFSP processes and CCMS/UniCORN is a plus.

Responsibilities

• Coordinate IFSP meetings, gather assessments, monitor progress and prepare reports.
• Advocate for families, connect them to SSI, Medicaid, and community resources.
• Coordinate family IFSPs, request funds, and monitor fund usage.
• Assist with Child Find activities and community outreach.
• Represent MVDS on committees and promote Part C requirements.
• Oversee Child Outcomes implementation with staff and families.
• Maintain accurate client records and timely case management entries.
• Collaborate with the Director to support program training and outreach.
