Job summary

Southwest Behavioral & Health Services seeks a dedicated Case Manager to coordinate care for children and adults in Prescott Valley. You will conduct assessments, connect members with community resources, and coordinate care inside and outside the agency.

The role offers competitive pay at $17.50–$18.00 per hour, with benefits including PTO, holidays, insurance, and a strong emphasis on teamwork and community impact.

Qualifications

• Must be at least 22 years old.
• Bachelor’s degree in behavioral health plus one year of relevant experience.
• A combination of experience and education totaling 2 years is acceptable.
• Valid AZ driver’s license & motor vehicle clearance.
• Fingerprint clearance issued by the Arizona Department of Public Safety or eligible to receive upon hire.

Job Preview At a Glance

The Case Manager serves as a central support within the interdisciplinary team, working with children and adults. This staff member conducts assessments, assists members with resources within the community and provides coordination of care inside and outside of the agency.

Pay Rate: $17.50-$18.00 hourly

Responsibilities

• Intake screenings (Completing intake paperwork and referrals)
• Completing annual behavioral health assessments, including service planning and support planning
• Facilitating CFTs and ARTs
• Facilitating groups
• Identifying gaps in care for our members
• Completing SMI and SED evaluation

Qualifications

• Minimum of 22-year of age
• Bachelor’s degree in the behavioral health field plus one year of relevant experience in behavioral health
• A combination of experience and education totaling 2 years is acceptable.
