Job summary

Wellmore Behavioral Health in Danbury, CT is seeking a Care Coordinator to support families served by Child First. You will connect children and families with community-based services, maintain a reflective practice, and collaborate with a Mental Health and Developmental Clinician.

The ideal candidate is highly organized, self-motivated, reliable, and flexible, willing to work some evenings, and eager to participate in reflective supervision and team learning.

Qualifications

• Bachelor’s degree in child development, psychology, nursing, human services, or related field.
• At least three years working with culturally diverse families and young children under six.
• Knowledge of early childhood development and family risk factors (poverty, homelessness, domestic violence, teen parenthood).
• Experience with or knowledge of trauma, mental health challenges, or substance use disorders in adults is a plus.
• Experience with community-based services and home visiting is valued.
• Strong written and verbal communication; proficient with Word and Excel.

Responsibilities

• Provide community resource expertise to Child First team and families, including identifying and collaborating with community-based service providers and supports.
• Engage with the Child First family and the Clinician in the collaborative family assessment process using data from interviews, observations, and standardized measures.
• Promote family stabilization by identifying and supporting needs and addressing barriers to services.
• Maintain a reflective stance when engaging with the caregiver to understand motivation and possible barriers to new services.
• Use videotaping to enhance therapeutic work and reflective supervision.
• Provide the family with interactive, growth-promoting play experiences.
• Engage in weekly supervised reflective sessions and participate in the Learning Collaborative.
