Job summary

The Youth Service Bureau of Illinois Valley seeks a Child and Family Team Meeting Facilitator to coordinate and lead CFTMs, developing strong permanency plans for clients via virtual meetings.

You will prepare notes, track CFTMs, and hold 3–5 meetings per day using Teams or Zoom, collaborating with caseworkers to identify needs.

The role emphasizes Foster Care Specialized cases, confidentiality, professional boundaries, and adherence to agency procedures.

Qualifications

• Master’s degree in social work, psychology, education or related field.
• Experience with Child Welfare and Licensure processes is a MUST.
• Sensitivity to cultural differences when working with diverse families.

Responsibilities

• Prep notes and facilitate Child and Family Team Meetings (CFTMs).
• Track when CFTMs are needed.
• Holds 3-5 CFTMs per day, ALL VIA Teams or Zoom (no in-person).
• Facilitates meetings with Caseworkers to identify needs.
• After CFTM, write a case note.
• Focused on Foster Care Spec Cases and contract deliverables.
• Maintains professional boundaries.
• Treats all participants and families equally.
• Follows agency procedures regarding interactions with participants.
• Maintains confidentiality of participant/agency information.
• Demonstrates professional, assertive communication with participants and supervisor.
• Understands and supports program goals/agency mission.
• Has read and understands policies and procedures.
• Establishes relationships with local community resources.

In coordination with the Family Finding and Child Welfare Director, the Child Family Team Meeting Facilitator will hold Child and Family Team Meetings to establish a strong permanency plan for clients.

*This position can be based out of one of our following offices: Ottawa, Aurora, Crystal Lake, Princeton, or Rockford.

Qualifications

• Master’s degree in social work, psychology, education or related field
• Previous experience and knowledge of Child Welfare and Licensure processes is a MUST
• Sensitivity and responsiveness to the cultural differences of individuals and families in groups outside one’s own
