Qualifications

Responsibilities

• Coordinate and collaborate on services for children 0–17.
• Assess family needs and begin person-centered planning.
• Provide referrals, linking and coordination of services.
• Deliver individual, group and family IDD sessions.
• Maintain caseload and documentation per guidelines.
• Maintain clinical records and deadlines.
• Participate in multidisciplinary teams and school advocacy.
• Collaborate with CMH and schools to align Medicaid goals.
• Build positive relationships with referral sources.
• Manage multiple priorities within team.
