Job summary

Claratel Behavioral Health in Decatur, GA, is seeking a Child & Adolescent Case Manager/Community Support Individual (CSI) to deliver recovery-focused, family-driven services in the community. You will perform intake assessments, coordinate care, participate in interdisciplinary teams, document thoroughly, and engage families using natural supports to achieve IRP goals.

Qualifications

• Bachelor’s degree in counseling, social work, or related field.
• At least two years’ experience in a behavioral health setting providing case management/community support.
• Proficient with EMRs, telehealth systems, and mobile devices.

Responsibilities

• Completes community needs and clinical assessments for all individuals on caseload.
• Participates in interdisciplinary treatment team meetings to provide input for client treatment needs.
• Provides ongoing case management and skill-building aligned with treatment plans to achieve goals.
• Documents all service contacts timely and accurately per standards.
• Maintains regular contact with the Program Manager to review case management and administrative issues.
• Performs outreach to clients at least twice monthly and coordinates care with community providers.
