Qualifications

• Bachelor's degree in a related field (counseling, social work, etc.).
• Two years' experience in behavioral health case management/community services.
• Proficient with EMR systems, computers, telehealth tools and mobile devices.
• Strong care coordination and resource linkage abilities.
• Reliable transportation and positive, collaborative teamwork.

Responsibilities

• Complete community needs and clinical assessments for caseloads.
• Participate in interdisciplinary treatment team meetings to inform care.
• Provide ongoing case management and skills-building per treatment plans.
• Maintain timely, high-quality documentation for all contacts.
• Engage with Program Manager for professional growth and admin duties.
• Outreach to clients at least twice monthly and coordinate with providers.
• Communicate with clinicians about client challenges and safety concerns.
• Transport participants for community integration outings as needed.
