The Role
This is not a traditional CTO hire. We're looking for a hands-on AI agent builder, someone who writes code daily, ships production AI systems, and can architect the next generation of autonomous clinical care agents, while carrying the CTO title and owning the full technology vision.
The company sits on a goldmine of real-time patient data (vitals, device telemetry, EMR records, clinical notes) flowing from 150,000+ monitored patients. The opportunity is to build intelligent agents that act on that data: triaging alerts, predicting deterioration, automating care coordination workflows, and augmenting clinical decision-making at scale. You'll be building this largely from scratch with a small, high-caliber team.
What You'll Build
- Autonomous Clinical Agents: Design and ship AI agents that monitor patient data streams in real time, elevate intelligently, and automate care coordination tasks that today require manual clinical effort.
- LLM-Powered Workflows: Build agent pipelines using large language models for clinical note summarization, patient communication, care plan generation, and intelligent triage.
- Predictive & Preventive Models: Develop and deploy models for risk stratification, readmission prediction, and early deterioration detection across chronic disease populations.
- Agent Orchestration: Architect multi-agent systems that coordinate across data ingestion, clinical reasoning, provider notification, and patient engagement, with appropriate human-in-the-loop safeguards for healthcare.
- Data Infrastructure: Build the real-time data pipelines and feature stores that feed your agents, patient vitals from connected devices, EMR data via HL7/FHIR, and unstructured clinical documentation.
- EMR Integration Layer: Extend and harden integrations across 50+ EMR/EHR systems to ensure agents can read from and write back to provider workflows seamlessly.
Who You Are
- You ship AI systems, not slide decks. You're writing Python/TypeScript daily, prototyping agent architectures, and pushing to production. You've built and deployed LLM-based agents, RAG pipelines, or autonomous workflows in a production environment.
- 5-10+ years in software engineering, with recent deep focus on applied AI/ML and agent systems (LangChain, AutoGen, CrewAI, custom agent frameworks, or similar).
- Fluent in the modern AI stack: LLMs (GPT-4, Claude, open-source models), vector databases, embedding pipelines, function calling, tool use, prompt engineering, fine-tuning.
- Strong software engineering fundamentals, you build reliable, observable, production-grade systems, not just notebooks.
- Experience with real-time data pipelines and event-driven architectures (Kafka, streaming, webhooks).
- Healthcare experience is a major plus, familiarity with HIPAA, HL7/FHIR, clinical workflows, and the sensitivity required when AI touches patient care.
- Comfortable leading a small engineering team (4–6 today) while staying deep in the code. You'll hire and mentor, but this is at least 60–70% hands-on for the foreseeable future.
- You understand the stakes of AI in healthcare, you think carefully about safety, hallucination mitigation, human-in-the-loop design, and regulatory constraints.
Nice to Have
- Experience with IoT / connected medical devices and wearable data streams.
- Background in population health, chronic disease management, or clinical decision support.
- Prior experience at a startup where you were the first or primary AI/ML hire and built the function from zero.
- Familiarity with FDA software guidance (SaMD) or clinical AI validation frameworks.
