Job summary

Leoforce seeks a Director of Quality/Risk Management to lead hospital-wide quality, patient safety, and risk programs at the Modesto, CA facility. The role oversees CMS and Joint Commission compliance, drives QAPI initiatives, and coordinates survey readiness with executive leadership.

Qualifications

• Bachelor’s degree in healthcare or related field
• Experience in Quality and/or Risk Management with primary responsibility for performance improvement, regulatory compliance, conflict resolution, leadership, and risk management
• Strong working knowledge of CMS guidelines and Joint Commission regulations (must-have for this search)
• Clinical background preferred; non-clinical candidates with substantial hospital quality/risk experience will be considered
• Hospital-based quality and/or risk leadership experience; IRF or post-acute experience is a plus

Responsibilities

• Lead hospital-wide quality management, patient safety, performance improvement, and risk management programs
• Ensure compliance with CMS guidelines, The Joint Commission, accrediting bodies, and corporate/hospital policies and procedures
• Develop, implement, and maintain quality assessment and improvement (QAPI) programs
• Assess compliance with federal, state, and industry regulatory and accreditation standards and drive remediation or sustainment plans
• Serve as primary local leader for Joint Commission, CMS, and other survey readiness, education, and on-site coordination
• Oversee risk management activities including incident reporting, notice of potential claims, corrective action planning, and reporting to Corporate Risk
• Coordinate review, development, and implementation of hospital policies
• Maintain and update hospital plans including Provision of Care/Scope of Services, Leadership, Information Management, Utilization Review, Infection Control, and Patient Safety
• Use quality applications and data sources (including IRF tools such as UDS, ORYX, Press Ganey, and similar systems) to identify opportunities, generate reports, and track outcomes
• Support and sustain disease-specific certification work in Stroke and Parkinson’s Rehabilitation
• Provide organizational education related to regulations, standards, and performance improvement
• Lead conflict resolution related to quality, safety, and risk issues
• Direct the PASC function as a reporting department
