HarmonyCares is seeking a VP and Senior Medical Director, Population Health to lead population health strategy and clinical program design for home-based care across multiple markets. You will align clinical priorities with measurable goals and scale successful models while working with executives and frontline teams.

The role requires MD/DO with an active license and board certification, with 8+ years in population health or related fields.

Qualifications

• MD or DO with active, unrestricted license.
• Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.

Responsabilités

• Lead population health strategy and clinical program design across HarmonyCares Medical Group.
• Translate data and field feedback into interventions for home-based care teams.
• Partner with Analytics, Finance, Actuarial to evaluate clinical and economic results.
