Job summary

HarmonyCares seeks a Chief Clinical Officer to translate our clinical strategy into a scalable home-based care model across markets. You will design workflows, educate teams, and drive quality improvement in close partnership with executive leadership.

Lead population health programs, foster physician collaboration, and guide clinical product development with an emphasis on safe, high-value care for complex patients in multiple states.

Qualifications

• MD or DO with active, unrestricted license.
• Board certification in Family, Internal or Geriatric Medicine.
• 10+ years leading interdisciplinary care models at scale.
• Experience turning strategy into workflows, roles and tools.
• Expertise in clinical operations and quality improvement.
• Strong Medicare/value-based care knowledge.
• Credible working with physicians and non-physician clinicians.

Responsibilities

• Design the home-based care model, including team roles and visit cadence.
• Translate evidence into practical pathways for frailty, transitions, and risk.
• Define enterprise-wide standards and local adaptation boundaries.
• Build routines for high-risk review, panel management, and care coordination.
• Lead clinical design of population health programs and test feasibility.
• Partner on Stars, HEDIS, safety, and utilization drivers.
• Set practice expectations, onboarding, and professional development.
• Plan workforce and clinician retention, performance improvement.
• Co-design workflows with Product/Tech and frontline testing.
• Oversee adoption of clinical tools, automation, and AI with safeguards.
