Job summary

Stand Out Home Care is seeking a Certified Home Health Aide (CHHA) to support clients in their homes across New Jersey. You will assist with daily living activities, provide companionship, and follow individualized care plans under supervision.

Qualifications

• Certified Home Health Aide (CHHA) license required (New Jersey).
• Ability to work independently with minimal supervision.
• Capable of following an individualized care plan and documenting care.

Responsibilities

• Assist clients with daily living activities such as bathing, dressing, grooming, toileting and mobility.
• Provide companionship and emotional support to clients.
• Perform light housekeeping, meal preparation and medication reminders.
• Follow the nurse-provided care plan and report changes in client condition.
• Maintain accurate and timely documentation of care.
