Vanderbilt University Medical Center is seeking a nursing professional to establish and maintain disease-specific networks with community and affiliate hospitals, ensuring adherence to national guidelines and certifying bodies. The role coordinates outcomes data and post-discharge follow-up, shaping patient care across populations.

You will collaborate with clinical leadership to meet program goals, mentor staff, and support education and training.

Qualifications

• Bachelor's degree in a relevant field or nursing equivalent preferred.
• Experience in clinical settings related to neuroscience or patient care is advantageous.
• Licensure as a Registered Nurse and ongoing professional development.
• Experience leading or coordinating clinical programs and teams.

Responsibilities

• Establishes and maintains a disease and/or patient population specific network with community and affiliate hospitals.
• Ensures TJC Certification/Accreditations and licensing requirements are maintained.
• collaborates with Leadership, Standards and Accreditations Department and Quality and Patient Safety Leadership.
• Maintains and develops protocols aligned with national practice guidelines and supports education for healthcare professionals.
• Facilitates Continuum of Care through community outreach and post-discharge follow-up.
