Job summary

Crestway Nursing and Rehabilitation Center in San Antonio, TX, is seeking a full-time LVN/LPN supervisor to oversee resident care and ensure adherence to facility procedures. Qualifications

• Active LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Nursing management experience is preferred but not required.

Responsibilities

• Follows established procedures for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions including nutrition, infections, skin issues, weight loss or other changes.
• Uses the Quality Life Manual for patient program identification, placement, and monitoring.
• Documents and notifies departments of patient room changes, LOA, new admits, or discharges.
• Might do rounds with physicians when necessary.

• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
