Job summary

Brentwood Place Four in Dallas, TX is seeking a dedicated RN/LVN to join our skilled nursing and rehabilitation team. The shift is 10 PM to 6 AM, and you will provide direct patient care with a focus on safety and quality.

Responsibilities

• Provide direct patient care, including assessment, planning, implementation, and evaluation of care plans
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Administer medications, treatments, and procedures as prescribed by physicians
• Monitor and document patient progress, reporting changes and concerns to the healthcare team
• Develop and maintain positive relationships with patients, families, and caregivers
• Participate in quality improvement initiatives to enhance patient satisfaction and outcomes
