Montefiore Medical Center in New York seeks a Registered Nurse (RN) to join its nursing team. BSN required; active NY State license; BLS and specialty certification on hire; 1 year of hospital or clinic experience preferred.

The RN will lead care coordination, develop interdisciplinary plans of care, document patient progress, and ensure timely teaching and discharge planning. Strong English communication and collaboration with the care team are essential.

• Attends at least one hospital committee meeting or in-service annually and shares information with coworkers.

Responsabilidades

• Demonstrates leadership in coordination and delivery of patient care.
• Coordinates an interdisciplinary plan of care and ensures 100% of assigned patients have a documented plan.
• Completes admission assessments within 8 hours and documents interventions.
• Maintains a therapeutic relationship with patient/family and participates in discharge planning.
• Communicates effectively with patient/family and health care team; documents care clearly.
