Job summary

Cantex Continuing Care Network seeks an LVN/LPN in a Skilled Nursing setting at Coronado at Stone Oak in San Antonio, TX. The PRN role supports patient care across shifts and ensures compliance with nursing policies.

You will observe patient conditions, administer treatments, document faithfully, and work with CNAs to maintain high-quality care for residents in a on-site facility.

Qualifications

• Graduation from an accredited Nursing School.
• Valid LVN/LPN license to practice.
• Must meet state/federal nursing requirements.

Responsibilities

• Oversee patient care within the LVN/LPN scope.
• Observe, assess, and report patient conditions.
• Document patient assessments and changes in EMR.
• Administer medications and treatments as prescribed.
• Supervise CNA staff to ensure quality care.

Qualifications: Responsibilities:

• As an LVN/LPN - Licensed Vocational Nurse or Licensed Practical Nurse, you will participate in and oversee patient care within the scope of the LVN/LPN licensure.
• Observes, assesses, and reports Patient conditions/changes and documents in accordance with Cantex’s Clinical Policies & Procedures and state/federal guidelines & regulations.
• EMR documentation relevant to patient assessment, evaluation, change of condition, in accordance to nursing practices.
• Ensures implementation of care plans as directed and administers medications and treatments as prescribed.
• Participates in assigning duties and supervises the delivery of appropriate clinical care by the CNA staff to ensure highest practical well-being is maintained by patient.

Requirements:
