Job summary

Silver Spring Health & Rehabilitation in Texas is seeking a Charge Nurse to lead direct patient care, monitor health, and support the healthcare team under supervision of RNs and physicians. You will ensure compassionate care in a fast-paced setting.

The role requires a current TX LVN/RN license, BLS, and experience in long-term care. You will collaborate with nurses, physicians, and staff to implement care plans, document accurately, and educate patients and families.

Qualifications

• Current and valid LVN/RN license in Texas.
• High school diploma or equivalent; completion of an accredited vocational nursing program.
• BLS certification from the American Heart Association or equivalent.
• Experience nursing in long-term care or rehabilitation settings.
• Strong communication and teamwork within a multidisciplinary team.

Responsibilities

• Provide direct nursing care to patients, including administering medications, monitoring vital signs, and assisting with activities of daily living.
• Collaborate with RNs, physicians, and other healthcare professionals to develop and implement individualized care plans.
• Document patient information accurately and timely in medical records, ensuring compliance with regulations and policies.
• Observe and report changes in patients’ conditions to the team promptly for appropriate interventions.
• Assist with wound care, catheter care, and other procedures as delegated by supervising staff.
• Educate patients and families on health maintenance, medications, and post-discharge care instructions.
• Maintain a clean, safe, and organized work environment in accordance with infection control standards.

About the Role:

As a Charge Nurse at Silver Spring Health & Rehabilitation, you will play a critical role in delivering compassionate and high-quality nursing care to our patients. Your primary focus will be to support the healthcare team by providing direct patient care, monitoring patient health, and assisting with medical procedures under the supervision of registered nurses and physicians. Minimum Qualifications:

• Current and valid LVN/RN license in the state of Texas.

• Additional certifications such as IV therapy, wound care, or geriatric nursing.
• Familiarity with electronic health record (EHR) systems and computerized documentation.
• Knowledge of state and federal healthcare regulations related to long-term care.
• Proven ability to provide culturally competent care to a diverse patient population.
