Qualifications

• Basic Life Support (BLS) certification from the American Heart Association or equivalent.
• Demonstrated ability to perform nursing duties in a long-term care or rehabilitation setting.
• Strong communication skills and ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Provide direct nursing care to patients, including administering medications, monitoring vital signs, and assisting with activities of daily living.
• Collaborate with registered nurses, physicians, and other healthcare professionals to develop and implement individualized care plans.
• Document patient information accurately and timely in medical records, ensuring compliance with healthcare regulations and facility policies.
• Observe and report changes in patients’ conditions to the healthcare team promptly to facilitate appropriate interventions.
• Assist with wound care, catheter care, and other clinical procedures as delegated by supervising medical staff.
• Educate patients and their families on health maintenance, medication management, and post-discharge care instructions.
• Maintain a clean, safe, and organized work environment in accordance with infection control and safety standards.

About the Role:

As a Charge Nurse at Silver Spring Health & Rehabilitation, you will play a critical role in delivering compassionate and high-quality nursing care to our patients. Your primary focus will be to support the healthcare team by providing direct patient care, monitoring patient health, and assisting with medical procedures under the supervision of registered nurses and physicians.
