Job summary

Treviso Transitional Care in Longview, TX seeks a dedicated LVN/RN for the night shift (6p-6a) to provide compassionate nursing care. You will administer medications, monitor vitals, and support residents with daily living activities under RN/physician supervision.

Strong communication and teamwork are essential to coordinate care plans and advocate for residents. Responsibilities include accurate charting, updating care plans, and educating residents and families.

Qualifications

• Current and valid Texas LVN/RN license.
• Completion of an accredited vocational nursing program.
• BLS certification.
• Experience in long-term care or rehabilitation setting.

Responsibilities

• Provide direct nursing care to residents, including medications and Treatments as prescribed.
• Monitor vital signs and changes in patient conditions; report significant findings.
• Assist with activities of daily living and mobility.
• Collaborate with nurses, physicians, and healthcare professionals on care plans.
• Maintain accurate patient records in accordance with policies and regulations.
• Educate residents and families on health maintenance and disease prevention.
• Ensure infection control and safety protocols are followed.

About the Role:

As a LVN/RN at Treviso Transitional Care , you will play a critical role in delivering compassionate and high-quality nursing care to our residents. Your primary focus will be to support the health and well-being of patients by administering medications, monitoring vital signs, and assisting with daily living activities under the supervision of registered nurses and physicians. You will collaborate closely with interdisciplinary teams to develop and implement individualized care plans that promote recovery and comfort. This role requires a strong commitment to patient advocacy, ensuring that each resident's needs and preferences are respected and met with dignity. Ultimately, your contributions will directly impact the quality of life and health outcomes of the individuals entrusted to our care.

Minimum Qualifications:

• Current and valid LVN/RN license in the state of Texas.
• Completion of an accredited vocational nursing program.
• Basic Life Support (BLS) certification.
• Demonstrated ability to perform nursing duties in a long‑term care or rehabilitation setting.
• Strong communication and interpersonal skills.

Preferred Qualifications:

• Experience working in a nursing home or rehabilitation facility.
• Familiarity with electronic health record (EHR) systems.

Responsibilities:

• Provide direct nursing care to residents, including administering medications and treatments as prescribed.
• Monitor and record vital signs, symptoms, and changes in patient conditions, reporting significant findings to the healthcare team.
• Assist residents with activities of daily living such as bathing, dressing, and mobility support.
• Collaborate with registered nurses, physicians, and other healthcare professionals to develop and update individualized care plans.
• Maintain accurate and detailed patient records in compliance with facility policies and regulatory requirements.
• Educate residents and their families on health maintenance and disease prevention strategies.
• Ensure a safe, clean, and comfortable environment for residents by adhering to infection control and safety protocols.
