Job summary

Holland Lake Rehabilitation & Wellness Center in Weatherford, TX is seeking an experienced RN/LVN leader to oversee daily nursing operations and guide a large care team this fall season.

Responsibilities include hands-on resident care, medication administration, identifying changes in condition, and contributing to the Resident Plan of Care with emphasis on accurate assessment and documentation.

Qualifications

• Must have active Texas RN/LVN license and CPR.
• Minimum 1 year experience in hospital or long-term care facility.
• Excellent interpersonal and written/verbal communication.
• Strong leadership and collaboration skills.

Responsibilities

• Provide hands-on care to residents based on the Plan of Care.
• Administer medications and treatments per physician orders.
• Identify changes in resident condition and evaluate care needs.
• Strong focus on assessment and documentation.
• Evaluate and participate in the development of the Resident Plan of Care.
• Monitor overall condition and ensure compliance with policies and procedures.

Responsibilities

• Provide hand on care to the resident based on the resident Plan of Care.

• Strong focus assessment skills as well as documentation skills.
• Must be able to evaluate and participate in the development of the Resident Plan of Care.
• Monitor the resident overall condition and provide care as required while maintaining compliance with the facility policy and procedures.

Qualifications

Requirements

• An active Texas RN/LVN License and nursing degree from an accredited school of nursing, college or university along with a CPR certification
• A minimum of one year experience in a hospital, long term care facility or other related health care facility.
