Rock Creek Health & Rehabilitation in Sulphur Springs, TX is seeking a Charge Nurse - LVN or RN to lead our nursing team and support resident well-being. You will supervise care delivery, ensure accuracy in medication administration, and implement individualized care plans.

This full-time role requires strong clinical skills, clear communication, and the ability to coordinate shift operations. You’ll partner with physicians and staff to maintain high-quality care and resident safety.

Qualifications

• Must hold an active RN or LVN license in good standing.
• Strong clinical, assessment, and documentation skills.
• Excellent communication and ability to organize workflows.
• Responds promptly to emergencies with attention to detail.

Responsibilities

• Direct care teams: guide CNAs to ensure appropriate resident care.
• Administer medications safely as prescribed.
• Implement and monitor resident care plans.
• Perform treatments as required for resident health.
• Transcribe physician orders accurately and promptly.
• Document resident care in charts and complete nursing assessments.
• Participate in shift reports for care continuity.
• Educate staff on nursing policies and resident needs.
• Maintain infection control and safety standards.

We're seeking a dedicated and reliable Charge Nurse - LVN or RN to join our growing team! Your Impact as a Charge Nurse - LVN or RN

In this role, you will:

• Direct Care Teams: Provide guidance to Nurse Aides to ensure appropriate care for residents.
• Administer Medications: Properly and safely administer resident medications as prescribed.
• Implement Care Plans: Execute resident care plans in compliance with company policies, federal guidelines, and physician orders.
• Perform Treatments: Deliver treatments as required to ensure resident health and comfort.
• Transcribe Orders: Accurately and promptly transcribe physician orders.
• Document Resident Care: Maintain timely and accurate documentation in resident charts and complete required nursing assessments.
• Collaborate During Shift Reports: Participate in shift reports to ensure seamless communication and care continuity.
• Educate Support Staff: Implement nursing policies and procedures, educating staff on the care needs of residents.
• Maintain Safety Standards: Promote a safe working environment by complying with infection control and body mechanic procedures.
• Address Staffing Needs: Respond to staffing shortages by arranging for replacement staff as necessary.
