Job summary

Cantex Continuing Care Network in Sugar Land, TX is seeking a Charge Nurse (LVN/LPN) to provide direct patient care and nursing leadership in a long-term care setting. You will oversee care delivery, administer medications, coordinate care plans, and ensure regulatory compliance.

The role requires an LVN/LPN license, at least one year of supervisory experience in healthcare, and strong English communication.

Qualifications

• Graduate of an accredited nursing school with a current LVN/LPN license in the applicable state.
• Minimum of one year of supervisory experience in a long-term care or healthcare setting preferred.
• Ability to read, write, and speak English.

Responsibilities

• Observe, assess, document, and report patient conditions and changes per policies and regulatory requirements.
• Maintain accurate EMR documentation of patient assessments and nursing activities.
• Consult with RNs and Nurse Managers regarding changes in patient condition and needs.
• Receive, transcribe, and execute physician orders using approved methods.
• Implement care plans and administer medications and treatments as prescribed.
• Assign duties to CNAs and monitor delivery of patient care services.
• Document accidents, incidents, and significant events per standards.
• Lead as a team leader, coach staff, and provide counseling as needed.
• Coordinate equipment, supplies, meds, and devices used in care delivery.
• Notify attending physicians of significant changes in patient condition.
• Maintain QA logs and support operation of clinical equipment.
• Promote a safe environment and support abuse prevention requirements.
