Job summary

Qualifications

• Must be a Licensed Practical Nurse (LPN) in good standing and licensed by Oklahoma.
• Minimum 1 year experience in Skilled Nursing Facility or Long-Term Care.
• Diploma from an accredited School of Nursing is required.
• Ability to read, write, and follow directions as required.
• Excellent communication and teaching skills with residents and families.

Responsibilities

• Provide direct nursing care to residents and supervise CNAs/CMAs.
• Record and chart vital signs, medications, and orders per policy.
• Coordinate with care teams to maintain residents' care plans.
• Participate in QA program and regulatory surveys as needed.
• Ensure infection control, discharge planning, and documentation.
