Job summary

Qualifications

• Must be at least 21 years old.
• Ability to communicate effectively in English, both verbally and in writing.
• Meet state health-related requirements.
• Maintain current professional license if LPN/LVN.
• Maintain current CPR certification if LPN/LVN.
• Maintain any other certifications as required by state or provincial regulations.

Responsibilities

• Lead the Wellness team to promote dignity and choice for residents.
• Administer or assist residents to self-administer medications and treatments.
• Measure vital signs and weight.
• Communicate with physicians to relay orders and test results.
• Ensure documentation in resident charts and nursing notes.
• Supervise Personal Care Specialists and assist in service plan updates.
• Follow home health, hospice, and emergency protocols.
• Attend onboarding, training, and team meetings.
