Charge Nurse - LVN/LPN - Monday to Friday 6A-2PM

As a Charge Nurse, you will be responsible for providing high-quality patient care and leadership in our skilled nursing and rehabilitation center. About the Role

• Providing direct patient care to our residents, including medication administration, wound care, and other treatments
• Leading a team of Licensed Vocational Nurses (LVNs) and Licensed Practical Nurses (LPNs) in the provision of patient care
• Coordinates patient care with other healthcare professionals to ensure continuity and comprehensiveness of care
• Develops and implements patient care plans in collaboration with the interdisciplinary care team
• Maintains accurate and up-to-date patient records and reports
• Identifies and reports any changes in patient condition or concerns to the supervising nurse
