Job summary

Avera Health is seeking a Charge Nurse for the Brady Health and Rehab facility in Mitchell, SD. The role features a rotating schedule with every third weekend/holiday and on-call rotation, and may include a sign-on bonus up to $10,000.

As part of a multidisciplinary team, you will provide licensed practical nursing care, complete resident assessments, administer medications safely, and participate in care planning to ensure high-quality, compassionate patient care that supports Moving Health

Qualifications

• Licensed Practical Nurse (LPN) with active state license required.
• Ability to work rotating shifts including every third weekend and holidays.
• Experience in long term care preferred (less than 1 year).

Responsibilities

• Performs all licensed practical nursing functions as directed by RN.
• Completes resident assessments and plans care.
• Administers medications safely per physician orders and facility policy.
• Participates in care planning and ensures care delivery is appropriate to resident needs.
• Maintains accurate clinical documentation and uses SBAR for physician communication.
