Responsibilities

SPIRITUAL CAREGIVER - Home Health & Hospice

Our skilled staff offer end-of-life care for patients wherever they call home, focusing on those with life-limiting diagnoses. As a Spiritual Caregiver, you play an essential role in supporting patients' life journeys.

Under the supervision of the agency Director, the Hospice Spiritual Care Provider is responsible for delivering spiritual and bereavement care to hospice clients, families, staff, and volunteers.

Essential Key Job Responsibilities
  1. Document assessment, planning, and coordination of spiritual care:
  2. Complete spiritual assessments documented in the electronic medical record
  3. Develop patient/family care plans with individualized goals and interventions
  4. Complete spiritual assessments within five calendar days of admission
  1. Meet spiritual care needs of patients and families:
  2. Make visits as per frequency order; notes should reflect ongoing assessment of spiritual needs
  3. Increase visits or contacts as patient status declines
  4. Provide spiritual and emotional support to patients and families
  5. Coordinate spiritual care with community clergy as directed by the plan of care
  1. Coordinate agency bereavement services:
  2. Complete initial bereavement assessment within 4-6 weeks of request
  3. Develop and document bereavement care plans
  4. Offer grief/loss counseling as indicated
  5. Coordinate bereavement mailings
  6. Conduct agency grief support groups
  7. Organize hospice memorial events and community grief education
  1. Facilities and coordinates in-services, presentations, and special events:
  2. Assist in providing education for hospice volunteers
  3. Coordinate and assist with special event development
  4. Provide spiritual education to hospice team
  5. Participate in committees addressing hospice issues and needs
Qualifications
  • Appropriate education for spiritual care providers; experience in bereavement support
  • Completion of pastoral care and bereavement training programs
  • CPR certification
  • Valid driver’s license and auto liability insurance per company policy
Preferred Qualifications
  • Knowledge of spiritual assessment and intervention techniques
  • Excellent verbal and written communication skills

Pay Range

$17.93 - $24.66 per hour
